Unfortunately, losing eyesight is a common problem, be it due to the process of aging or the development of a specific condition. The good news is that 80% of visual impairment can be prevented or cured, but what of the remaining 20%?
In this Spotlight article, we take a brief look at retinal degeneration disorders - a group of related conditions that are presently without a cure. What treatments are currently being developed? And can researchers envision a future where sight can be restored to all patients?
Curable and incurable disorders
Most frequently, visual impairment is caused by uncorrected refractive errors (43%) or cataracts (33%). Refractive errors include myopia (short-sightedness), hyperopia (far-sightedness) and astigmatism, whereby the cornea or lens does not have a perfectly curved shape.
When visual impairment is caused by these problems, often treatment is readily available. Refractive errors can be corrected with glasses, contact lenses or refractive surgery. Cataracts - the clouding of the lens - are commonly treated with a surgical procedure that is among the most frequently carried out in the US.
While 80% of visual impairment can be prevented or cured, there remains 20% of cases for which there is currently no way of curing. A range of conditions exists where those who develop them are faced with a gradual loss of vision until their impairment is so severe that they are effectively blind.
Retinal degeneration disorders have no cure. These diseases break down the retina, the layer of tissue found at the back of the eye containing cells that detect light entering the organ.
There are a number of these degenerative diseases, including retinitis pigmentosa, macular degeneration and Usher syndrome. In particular, age-related macular degeneration is the leading cause of blindness in the developed world.
Medical News Today asked Dr. Raymond Iezzi, an ophthalmology consultant with the Mayo Clinic, what the biggest obstacles were to finding a cure for retinal degeneration disorders. He told us that scientists and clinicians face many challenges in developing treatments as there are several hundred biochemical abnormalities underlying theses disorders.
"Further," he added, "while there are several patterns of retinal degeneration, each is treated differently depending on the cells affected as well as the stage and severity of their degeneration."
When retinal degeneration conditions were first diagnosed, they were all labeled as retinitis pigmentosa. As knowledge in this area has improved, scientists have become aware that there is a variety of different related conditions, each affecting different areas of the retina with their own specific mechanisms.
In patients whose vision is still good, therapeutic approaches can be directed at neuroprotection or gene therapy.
"By protecting cells within the retina from death associated with the underlying biochemical disorder, we may preserve sight among large populations of patients," explained Dr. Iezzi. "A robust neuroprotection strategy would prevent cell death and vision loss, regardless of the underlying biochemical abnormality."
Gene therapy focuses instead on correcting the biochemical abnormalities that lead to the death of retina cells. This approach is highly specific, and Dr. Iezzi told MNT that several hundred treatments would need to be developed in order to treat the full range of retinal degenerative diseases.
Restoring and protecting vision with stem cells
The eye lends itself to experimental treatments, being easy to operate on and often protected from inflammatory responses that could disrupt therapies. Due to its accessibility, surgeons can also easily observe and track how new treatments are progressing.
Dr. Iezzi stated that at Mayo Clinic they are currently working on new methods to grow stem cells from a patient's own tissue samples; a regenerative approach that could one day lead to the restoration of sight to people that have lost it.

Macular degeneration is a major cause of partial blindness among people aged over 50.
Embryonic stem cells could be used to build new retinal pigmented epithelial cells - cells that nourish retinal visual cells and absorb light - that could be transplanted into a patient.
Doing this could slow or prevent the loss of the visual cells, and while deriving new visual cells from embryonic stem cells could lead to even more pronounced results, researchers have found it more difficult to successfully derive these cells and transplant them into the retina.
Mouse studies have previously shown that this technique can work and that transplanted cells can integrate fully with the retina, restoring vision to the animals.
There are two types of photoreceptive cells in the retina - rod cells and cone cells. While rod cells are stimulated by light over a wide range of intensities, perceiving shape, size and brightness, it is the cone cells that perceive color and fine detail.
Researchers have managed to derive rod cells from embryonic stem cells and are currently working on deriving cone cells and transplanting them into animals. If these trials prove successful, the next step could be human trials.
Without adequate neuroprotection, however, newly transplanted cells may be just as vulnerable to retinal degeneration disorders as the cells they replace. This problem underlies the importance of research into gene therapy aiming to correct the biochemical abnormalities that lead to cell death.
There is a form of treatment, however, that has led to the restoration of vision in patients with advanced forms of retinal degeneration disorders. Just as prosthetics can be used to restore function to individuals that have lost limbs, so too can individuals who have lost their vision use retinal prostheses.
The miracle of retinal prosthesis
"In patients who have already lost their sight, our therapeutic goal is to restore vision," said Dr. Iezzi. "This has been successfully accomplished via the Argus II retinal prosthesis in patients with advanced retinitis pigmentosa."
Last month, MNT reported on the story of Allen Zderad, a man who was effectively blind but is now able to make out the outlines of objects and people thanks to his new retinal prosthesis.
Dr. Iezzi was the ophthalmologist that put Mr. Zderad forward for the procedure, making him the 15th man in the US to receive the life-changing device. He is now able to navigate through crowded environments - such as shopping centers - without the use of a cane.
A camera connected to a pair of glasses transmits visual information to a small chip attached to the back of the eye via a small computer worn in a belt pack. The chip can send light signals directly to the optic nerve, bypassing the damaged retina and providing the patient with visual information in the form of flashes of light.
Dr. Iezzi describes what the retinal prosthesis provides as "artificial vision," unlike any form of vision that patients will have experienced before. While this form of vision could be considered basic compared with what normal-sighted people are used to, it is a marked improvement for many without sight.
As he used his retinal prosthesis for the first time, Mr. Zderad described the artificial vision as "crude, but significant."
"While this technology restores rudimentary vision, eventually as these devices improve, we may one day be able to treat patients with advanced macular disease such as Stargardt macular dystrophy or age-related macular degeneration," Dr. Iezzi suggested.
The future prospects of such treatment are truly exciting, but it is likely to be some time before the benefits of this technological breakthrough are widely experienced. Currently, the device costs $144,000, and the WHO estimate around 90% of the world's visually impaired live in low-income settings.

Dye-Coated Retinal Prosthesis Shows Promise for Restoring Vision

Researchers at Okayama University in Japan are planning to launch a clinical trial later this year for an innovatively simple retinal prosthesis they’ve developed to restore vision in people blind from retinitis pigmentosa (RP) and related conditions. Known as the Okayama University-type retinal prosthesis, or OUReP™, it’s a plastic film coated with a light-sensitive dye that is implanted beneath the retina. In a lab study, the device preserved vision in rodents with advanced RP. The investigators published the study’s results in the Journal of Artificial Organs.
Often referred to as artificial or bionic retinas, prostheses are designed to replace photoreceptors, the cells in the retina that process light and make vision possible. People blind from advanced retinal diseases have lost most or all of their photoreceptors.
Most retinal prosthetic technologies, including the Argus II and the Alpha IMS, use video cameras to capture images. An array of electrodes, implanted adjacent to the retina, then convert the images into electrical signals, which are sent to the brain, where they’re interpreted as vision.
The OUReP is much simpler. There is no camera or electrodes, and it does not need an electrical current, which usually requires an external power source. Instead, the dye on the film captures light coming into the eye and stimulates surviving retinal neurons, including ganglion and bipolar cells, to send visual information back to the brain.
Toshihiko Matsuo, M.D., Ph.D., the lead investigator on the project, says implanting the OUReP beneath the retina should be relatively straightforward in humans. “The sheet can be implanted in the subretinal space, using surgical methods established more than 20 years ago,” he explains. “Therefore, the implantation procedure should be technically feasible for vitreoretinal surgeons.”
He adds that the prosthesis’ polyethylene film has been used safely in other medical implants for humans, and the dye has not shown any toxicity in testing thus far.
In addition to preserving vision, the OUReP appears to reduce the loss of retinal cells in the rodents, suggesting that the dye may be protective. However, the investigators caution that more research is needed to better understand the effect.
“The simplicity of this approach is very attractive. Fewer components mean reduced risk for failures and complications,” says Stephen Rose, Ph.D., chief research officer, Foundation Fighting Blindness. “With that said, we need to see if the Okayama prosthesis can restore meaningful vision in humans. That’s the bottom line.”

Research led by Nicolas Bazan, MD, PhD, Boyd Professor and Director of the LSU Health New Orleans Neuroscience Center of Excellence, discovered a protein in the retina that is crucial for vision. The paper reports, for the first time, the key molecular mechanisms leading to visual degeneration and blindness. The research reveals events that may be harnessed for prevention, as well as to slow down progression of retinal degenerative diseases. The paper is published in Nature Communications.
There is growing evidence of the significance of the essential omega-3 fatty acid family member, docosahexaenoic acid (DHA), for photoreceptor function and in retinal degenerative diseases, but not much understanding about what governs it. The research team found that the protein receptor for adiponectin, a hormone that promotes insulin sensitivity and is involved in the metabolic syndrome, has a heretofore unrecognized function. The receptor also regulates DHA retention and conservation in cells in the eye and is necessary for photoreceptor cell function.
"This is the first time that such an integral membrane protein has been localized in the photoreceptor cells and shown to have the capacity to support sight," notes Dr. Bazan, the paper's corresponding author.
Working with a novel genetic mouse model they developed with the adiponectin receptor gene deleted, the researchers found that total and free retinal DHA were diminished in the gene-deficient mice. When they incubated normal retinas with labeled DHA, they measured abundant levels of it, demonstrating that a functional AdipoR1 gene must be present for DHA uptake and retention. Additionally, when cultured human Retinal Pigment Epithelial (RPE) cells were incubated with labeled DHA, DHA within the medium decreased with time while increasing within the cells. Also, when the AdipoR1 gene activity was ramped up in these cultured RPE cells, much more labeled DHA was taken up and incorporated. But when silenced, labeled DHA was diminished, indicating that human RPE cells can also take up DHA and that the AdipoR1 gene plays a significant role in this activity, too.
DHA in brain and retinal cells also builds reservoirs for molecules called into action when normal functions are disrupted, resulting in such conditions as retinal degeneration, Parkinson's or Alzheimer's disease. Dr. Bazan and his colleagues previously discovered neuroprotectin D1 (NPD1), one such molecule made from DHA when cell survival is compromised. Loss of, or diminished, retinal DHA leads to visual impairment and may play an important role in the development of blindness from retinitis pigmentosa and other retinal degenerative diseases, as well as age-related macular degeneration (AMD), the foremost cause of blindness in people older than 50 years.
"Our model and newly discovered molecular mechanism allow therapies to be tested more rapidly," notes Dr. Bazan. "We feel an urgency to address blindness and cognition impairments of dementias because of their heavy burden on patients, families, care givers and the health care system."
DHA, found in fish oil, is an essential omega-3 fatty acid and is vital for proper brain function. It is also necessary for the development of the nervous system, including vision. Dr. Bazan has been a pioneer in the understanding of the biology and impact of DHA in medicine.

There are about 48,000 corneal transplants done each year in the U.S., compared to approximately 16,000 kidney transplants and 2,100 heart transplants 12. Out of the 48,000 corneal transplants done, 10 percent of them end up in rejection, largely due to poor medication compliance. This costs the health care system and puts undue strain on clinicians, patients and their families. Johns Hopkins Medicine researchers may have discovered a way to prevent rejection by using biodegradable nanoparticles that release needed medication into the eye after surgery. This discovery could solve the decades-old issue of medicine compliance and help patients achieve corneal transplant success. "Medicine compliance is a major challenge in patient care," says Walter Stark, M.D., chief of the Division of Cornea, Cataract and External Eye Diseases at Johns Hopkins. "About 60 to 80 percent of patients don't take medicine the way they are supposed to." In an animal study being published in the Journal of Controlled Release, researchers looked into ways to alleviate the strain of adhering to a post-surgery treatment regimen that is sometimes hard to manage. Rats that underwent a corneal graft surgery were randomly divided into four groups and were given various treatments. One group was injected weekly for nine weeks with a safe, biodegradable nanoparticle loaded with corticosteroids for timed release of medicine. The other three groups received weekly injections of saline, placebo nanoparticles and free dexamethasone sodium phosphate aqueous solution after surgery, respectively. Treatments were given until the graft was clinically deemed as failed or until the nine-week test period concluded. Researchers looked at corneal transparency, swelling and growth of new blood vessels to decide if a graft had failed. For rats that received the nanoparticle loaded with corticosteroids, 65 percent of the treatment remained in the eye and did not leak within one week of the surgery. The concentration of the treatment also remained stronger than in the other three treatment groups. Additionally, there were no signs of swelling, and the cornea was clear throughout the test period. There were also far fewer instances of unwanted growth of new blood vessels in this group. Two weeks after surgery, rats that received the placebo nanoparticle and saline injections had severe swelling, opaque corneas and unwanted growth of new blood vessels, all indicating graft failure. After four weeks, rats that received free dexamethasone sodium phosphate aqueous solution all had graft failure as well. The only group that showed successful corneal transplant was the group of rats that received the corticosteroid-loaded nanoparticle injections. The grafts were still viable in 100 percent of these rats. "Corneal grafts are not easy to come by, and a lot of testing and time goes into ensuring the safe use of a graft for cornea transplant," says Qingguo Xu, Ph.D., a research associate at the Center for Nanomedicine at the Wilmer Eye Institute at Johns Hopkins Medicine. "This is why we want to do a better job at making sure corneal transplants don't end up in rejection, and our study illustrates a potentially better way." The steroid-loaded nanoparticle treatment group showed no signs of corneal transplant rejection. "That's 100 percent efficacy, a very promising finding," says Justin Hanes, Ph.D., director of the Center for Nanomedicine. "This type of treatment may also help prevent corneal transplant rejection in humans while making medicine adherence much easier on patients and their families." The nanoparticle loaded with medication could eliminate the need for a patient to remember to take their medicine - often multiple doses per hour - after a surgery, alleviating compliance risk. These types of drug delivery systems could be paired with other drugs and used in other conditions, such as glaucoma, macular degeneration and corneal ulcers, among others. The research team intends to continue the collaboration between engineering and medicine to look further into better ways to treat eye diseases. |

Ophthalmologists encourage organ donation to help restore sight for thousands
The number of corneal transplants needed to restore vision keeps growing each year. Nationwide, ophthalmologists performed more than 48,000 of these procedures in 2013, about 10,000 more than five years prior.[1] As this need continues to increase, organ donors who provide the eye tissue that makes these sight-restoring operations possible will become even more important.
In support of Eye Donor Awareness month this March, the American Academy of Ophthalmology is encouraging eye donation for corneal transplants by highlighting the importance of this life-changing surgery.
Often called the window of the eye, the cornea is the clear, smooth layer on the front of the eye that bends light, allowing people to see. Sometimes the cornea becomes cloudy or rough due to an eye injury, infection or a medical condition, such as Fuchs' Dystrophy. A corneal transplant can help restore vision by replacing the damaged cornea with one from an organ donor.
Lorie Gordon needed a new cornea because of keratoconus, a condition in which the cornea bulges, causing distorted vision. Her sight deteriorated so rapidly in her left eye that she needed a prescription for her glasses every six to eight months, and worried that failing eyesight would prohibit her from driving and working. After consulting her ophthalmologist, a medical doctor who specializes in the diagnosis, medical and surgical treatment of eye diseases and conditions, she underwent a corneal transplant to get a healthy cornea. Now, Gordon still needs glasses, but she can see well enough to read books and magazines, and keep her job as a teacher's assistant.
"I think it's a miracle," said Gordon, now age 49. "I'm able to see again and there's no price you can put on that."
Patients like Gordon receive donated corneas from their local chapter of the Eye Bank Association of America. The Eye Bank is the country's largest network for recovering, storing and distributing eye tissue and corneas for donation, relying on organ donors who sign up through their state licensure program or a donor registry. More than 95 percent of all corneal transplant operations will restore vision in the recipient, making corneal transplants some of the most successful types of transplantation in medicine, according to the Eye Bank.
"Corneal transplants have been able to restore vision to people who previously had no hope," said cornea specialist Stephanie Marioneaux, M.D., a clinical spokesperson for the American Academy of Ophthalmology and Gordon's ophthalmologist. "They can drive again, go back to work and live more fully thanks to eye donors, whose legacy will forever include helping others regain the precious gift of sight."

Gold nanoparticles enable precise stimulation of normal, non-genetically modified neurons with light
Light can be used to activate normal, non-genetically modified neurons through the use of targeted gold nanoparticles, report scientists from the University of Chicago and the University of Illinois at Chicago. The new technique, described in the journal Neuron, represents a significant technological advance with potential advantages over current optogenetic methods, including possible use in the development of therapeutics toward diseases such as macular degeneration.
"This is effectively optogenetics without genetics," said study senior author Francisco Bezanilla, PhD, Lillian Eichelberger Cannon Professor of biochemistry and molecular biology at the University of Chicago. "Many optogenetic experimental designs can now be applied to completely normal tissues or animals, greatly extending the scope of these research tools and possibly allowing for new therapies involving neuronal photostimulation."
Optogenetics, the use of light to control neural activity, is a powerful technique that has seen widespread use in neuroscience research. It involves genetically engineered neurons that express a light-responsive protein originally discovered in algae. This allows scientists to stimulate individual neurons as well as neural networks with precise flashes of light. However, since optogenetics is reliant on genetic modification, its use is primarily limited to relatively few model organisms.
Bezanilla and his colleagues have previously shown that normal, non-genetically modified neurons can be activated by heat generated from pulses of infrared light. But this method lacked specificity and can damage cells. To improve the technique, they focused on gold nanoparticles - spheres only 20 nanometers in diameter, more than 300 times smaller than a human blood cell.
When stimulated with visible light, spherical gold nanoparticles absorb and convert light energy into heat. This heating effect, which is most efficient using green light, can activate unmodified neurons. However, nanoparticles must be extremely close to a cell to produce any effect. Since the nanoparticles diffuse quickly, or get washed away in a neuron's immediate environment, their efficacy is short-lived.
To get nanoparticles to stick, Bezanilla and his team coupled them to a synthetic molecule based on Ts1, a scorpion neurotoxin, which binds to sodium channels without blocking them. Neurons treated with Ts1-coupled nanoparticles in culture were readily activated by light. Untreated neurons were non-responsive. Importantly, treated neurons could still be stimulated even after being continuously washed for 30 minutes, indicating that the nanoparticles were tightly bound to the cell surface. This also minimized potentially harmful elevated temperatures, as excess nanoparticles were washed away.
Neurons treated with Ts1-coupled nanoparticles could be stimulated repeatedly with no evidence of cell damage. Some individual neurons, targeted with millisecond pulses of light, produced more than 3,000 action potentials over the span of thirty minutes, with no reduction in efficacy. In addition to cultured cells, Ts1-coupled nanoparticles were tested on complex brain tissue using thin slices of mouse hippocampus. In these experiments, the researchers were able to activate groups of neurons and then observe the resulting patterns of neural activity.
"The technique is easy to implement and elicits neuronal activity using light pulses. Therefore, stimulating electrodes are not required," Bezanilla said. "Furthermore, with differently-shaped nanoparticles it can work in near-infrared as well as in visible wavelengths, which has many practical advantages in living animals. Thus far, most optogenetic tools have been limited to visible wavelengths."
While Ts1 was effective, it did not allow the stimulation of non-Ts1-responsive neuronal populations. To develop a more general strategy of cell targeting, the researchers coupled nanoparticles to antibodies that target other highly expressed proteins in neurons. They chose two antibodies that bind the ion channels TRPV1 and P2X3. Similar to Ts1, neurons treated with these antibody-coupled nanoparticles were activated by light even after continuous washing.
That nanoparticles can be coupled to different antibodies and retain efficacy suggests flexibility for future applications, including human therapeutic development. In retinal diseases such as age-related macular degeneration, for example, photoreceptor cells that absorb light signals are damaged or dead. However, the retinal nerve cells that carry visual information to the brain often remain intact and healthy. Nanoparticles targeted to these cells could potentially absorb light and directly stimulate the neurons, bypassing defective photoreceptors, according to the authors.
"While much additional research must be done to determine the feasibility of this nanoparticle approach as a vision restoration therapy, our results encourage further effort aimed at achieving this critical clinical objective," said study co-author David Pepperberg, PhD, Searls-Schenk Professor of ophthalmology and visual sciences at UIC.
Although no harmful effects were observed, the team notes that toxicity is a possibility. However, many live-animal tests and human clinical trials have already been completed using formulations of gold nanoparticles without serious side effects. The researchers are now testing the efficacy of the technique in animal models to verify its potential for therapeutic use.
